Children with Disabilities Occupational Therapy feedback

Thank you for agreeing to take part in the survey. This is survey should take between five and ten minutes to complete.

As part of this survey, your Occupational Therapy team at the council, are collecting your social care reference number. This is so we can discuss any problems you might raise. We will only get in touch if we need to.

Our report will be kept anonymous.

Your information is only shared with third parties when needed and when the law allows. 

We will only keep your information for as long as is necessary, and in line with our retention schedules(External link).

If you are logged in when completing the survey, your user account details will be recorded against your responses in addition to the personal data you provide within the survey. 

Your user account details will be removed before survey responses are analysed.
