We would like your feedback on this process to understand what is working well and what needs to be improved.
The new process for referring and reporting on advocacy came into effect in January 2023. Following a Safeguarding Adult Review (LLC) a recommendation was made for Adult Care to review the recording practice of advocacy involvement. A task and finish group was set up working alongside Voiceability to create a new process.
This is a new mandatory step to evidence and monitor referrals to the advocacy service. Practitioners should now be using the online referral form on Voiceability’s website and then completing a short form in Mosaic called “Adult Advocacy Monitoring”.
