We’d like your help to improve the NHS Health Check service in Lincolnshire. Records show that you were invited to have an NHS Health Check. We would like to understand the reasons why you didn’t attend your NHS Health Check so we can improve the service in the future.

Your responses to this survey are intended to be anonymous. When completing open text responses, please remember not to include personal or sensitive information in your response. 

Due to the nature of the Let’s Talk platform, please be aware that if you are logged in at the time of completing the survey, your user account details will be recorded against your responses.
