This survey is meant to be anonymous. That means we don’t need or want to know who you are – we won’t try to identify you from your answers. 

Please don’t write any private details in your answers to the questions where we ask for your views (where there is no tick box). 

If you are logged in to Let’s Talk, your Let’s Talk account information will be linked to your answers. These details are removed before we look at the results.
