Trusted Assessor Survey

If you are logged in when completing the survey, your Let’s Talk Lincolnshire (LTL) user account details will be recorded against your responses in addition to the personal data you provide within the survey.

If you do not want your LTL user account details associated with your responses, you should log out of your account before continuing with the survey. We only use the information you provide for the purposes described above. We will only ask you to provide the minimum amount of information we need, to do this. As well as the information provided by you, we may also use information from our internal records about relevant services you receive. Your information is only shared where necessary and where the law allows it.  We may share some anonymous quotes from survey responses with Homecare Providers, so they can be published on their websites. Usually, reviews are completed with Adult Social Care so we are trying to understand how people feel about their reviews being carried out by Providers instead. 

Most of the questions are on a 1-10 rating scale, where 1 = Strongly Disagree and 10 = Strongly Agree. 

There are also some open text questions for you to expand on your experience in your own words.
